ANNCESTOR SHEET (FORM A) INSTRUCTIONS This form is used to list a person's ancestors (spouse, parents, grandparents, great grandparents, etc.). Typically, the spouse marrying into the ROLL family would fill out this form; so we would know who are his/her ancestors (parents, grandparents, etc.). It would not be necessary to fill out the ancestors on the ROLL side because we already researched the ROLLs back a few centuries. We are particularly interested in receiving Ancestor Sheets on spouse's parents who are of German-Russian origin! You can download forms and form instructions at www.rollintl.com/roll/forms.htm. ANCESTOR SHEET OVERVIEW: - a) Print a bold asterisk (*) by the name of each person you filled out a Family Group Sheet. - b) Make sure to use the proper name, date, and place formats. ANCESTOR SHEET INSTRUCTION NUMBERS EXPLAINED: (see attached numbered form A) 1. DATE: Write the date this form was completed using the proper date format: Day MONTH Year (16 Feb 1956). 2. F# (Family Group Sheet Number): Leave this box blank. It will be used by the genealogist preparing the ROLL database. 3. COMPILED BY: Write the full name of the person who prepared this form using the proper date format: Day MONTH Year (16 Feb 1956). 4. ADDRESS: Write the preparer's full address including city, state and zip code. 5. PHONE: Write the preparer's home phone number with area code. 6. E-MAIL: Write the preparer's computer e-mail address. You may list more than one e-mail if it is necessary. 7. 1. {1st Generation - Male or Female} This is person #1 who you will list four generations of ancestors. Write his or her full birth name using the proper name format: First, "Nickname", Middle, LAST. 8. B: (Birth) Write the birth date using the proper date format: Day MONTH Year (16 Feb 1956). Estimate if necessary using proper date abbreviations. 9. PL: (Place) Write the place were the person was born using the proper place format: Town, County, State, Country. 10. M: (Marriage Date) Write the married date using the proper date format: Day MONTH Year (16 Feb 1956). Estimate if necessary using proper date abbreviations. 11. PL: (Place Married) Write the place were the couple married using the proper place format: Town, County, State, Country. 12. D: (Died Date) Write the death date using the proper date format: Day MONTH Year (16 Feb 1956). Estimate if necessary using proper date abbreviations. Additionally, you can list the cemetery were they were buried (or state that they were cremated). 13. PL: (1Place Died) Write the place were the person died using the proper place format: Town, County, State, Country. 14. REL: (Religion) Write the religion the person/family practiced. (Catholic, Lutheran, etc.) 15. SPOUSE: {1st Generation - Male or Female} This is person #2 who is married to person #1. Write the full birth name using the proper name format: First, "Nickname", Middle, LAST. 16. FATHER: {2nd Generation - Male} Write the full birth name of person #2's ("Spouse") father using the proper name format: First, "Nickname", Middle, LAST. 17. MOTHER: {2nd Generation - Female} Write the full birth (maiden) name of person #2's ("Spouse") mother using the proper name format: First, "Nickname", Middle, LAST. 18. C#: (Number of Children) Write the total number of children person #1 and #2 had in their family. Include adoptions and stillbirths. Exclude children from a previous marriage. 19. 2. {2nd Generation - Male of Female} This is person #2. He is the birth father of person #1 (1st generation).
