GENERAL INSTRUCTIONS Please send all completed forms to either your Team Leader and/or Mitch Roll via mail, fax, or e-mail (see bottom of page). Your Team Leader will give you specific instructions, and should have their contact information written on each form. You can download forms and form instructions at www.rollintl.com/roll/forms.htm GENERAL OVERVIEW: Please take every reasonable step to see that the information on all forms is as accurate and complete as practical. This will help maintain the integrity of our database and genealogy book. You can write any miscellaneous information or additional detail by numbering each note with enclosed parenthesis {"(1)", "(2)", "(3)", etc.}, and then write further information on 1) unused portions of the form; 2) on the reverse side; or 3) on an attached sheet(s). Names: - Write given (birth) names using the following format: [First "Nickname" Middle LAST]. Example: John "Johnny" Michael ROLL - If a person was known by a nickname or by more than one name, write the other name with quotation marks ("") or write "or" between the names. A "People used to call her AMI" example: Anna BERGER or Ami Dates: - Write complete dates if known using the following format: [16 Feb 1956 ]. - Calculate unknown dates if possible. For example, a person who was two years old in 1910 was born in 1908. - APPROXIMATE the year if necessary by adding abt (about). Example: abt 1890. Sometimes you will see genealogist using ca which is the Latin form of the "about" abbreviation. - Write bef (before) if you know an event happened BEFORE a certain date. Example: bef 1905. - Write aft (after) if you know an event happened AFTER a certain date. Example: aft 1925. - If you know the DATE RANGE of an event, write bet (between) in front of the dates which are separated by a dash (-). Example: bet 1918-1920. - If you are sure of a DEATH but do not know the date, write D or dead. Places: - Write names of places in ORDER from smallest of largest geographical divisions, separated by commas (,). For example: Town, County, State, Country. Example: Dickinson, Stark, ND, USA. - If you do not know the place of an event, write of and the family's place of residence at the time of the event. Example: of Dickinson, Stark, ND - If you are purely guessing or have a hunch as to the place, put question marks (?) around the town, state, or country. Example of guessing a city:?Dickinson?, ND.
